GET CERTIFIED BY A PHYSICIAN

Visit an Ohio-certified physician who can confirm that you have a qualifying condition and have the physician create your profile in the patient and caregiver registry.
When you visit the doctor, be sure to take government-issued identification, including minors.

Acceptable forms of ID:

Unexpired Ohio driver’s license
Unexpired Ohio identification card issued by the Ohio BMV (adults & minors)
Unexpired US passport or US passport card (adults & minors)

CAREGIVER INSTRUCTIONS

In order to expedite the caregiver registration process and reduce exposure to higher risk patients during the COVID-19 outbreak, the State of Ohio Board of Pharmacy issued the following guidance for caregiver registration:

A patient can now have up to three caregivers, and caregivers can have up to three patients.
Caregivers may now be added by Board staff using the process outlined in the link below.

This guidance is being issued in accordance with a Board resolution adopted on March 2, 2020, and shall remain in effect until rescinded by the Board.
